Healthcare Services
We partner with Covenant Home Health to provide Medicare-certified outpatient rehabilitation, physical therapy, skilled home health, and supportive services to our residents. With an office located on-site and a team of staff dedicated to the Fellowship Square Phoenix community, residents have a wide range of healthcare services right outside their door.
In-Home Health Care
If you have been newly discharged from the hospital, or are having changes in your medical condition, we can provide skilled home health services in the comfort of your own home. To qualify for Skilled Home Healthcare, patients must have a physician’s order indicating medical necessity for the skilled services of a nurse or therapist.
If you don't already have doctor's orders, we will coordinate with your doctor to obtain them and verify your insurance benefits. After all, your job is to focus on improving your strength and well-being!
In-Home Supportive Services
Services are provided by certified caregivers or licensed CNAs in the comfort of your own home. Our caring staff can assist with activities of daily living such as bathing, dressing, medication set-up, light housekeeping, meal preparation, and laundry.
Supportive Services are private pay and do not require doctor's orders.
Outpatient Rehab & Physical Therapy
With an office located right on campus, residents can take advantage of the benefits of therapy without having to leave our campus. We are pleased to provide door-to-door transportation via golf cart at no extra charge. Services include physical, occupational & speech therapies.
